As the Health and Wellness Director, you will oversee all aspects of the care department, leading a high-performing team of floor nurses, a Resident Care Coordinator, medication technicians, and caregivers. This role is central to ensuring our residents receive top-tier service in an environment defined by respect, dignity, and personalized care. Position Overview: Health and Wellness Director | LVN | RN | LPN
Purpose:
To provide strategic, clinical, and operational leadership for resident care services. The HWD ensures that all care is delivered safely, effectively, and in alignment with California state regulations, resident rights, and industry best practices for Assisted Living and Memory Care (including communities with dementia-care endorsements).
Minimum Requirements
• Licensed Nurse (RN, LVN, LPN); strong background in long-term or community-based care preferred
• Bachelor’s degree preferred
• Current California nursing license in good standing
• Valid driver’s license and reliable transportation for off-site assessments and outreach
• Deep knowledge of dementia-care best practices and communication techniques
• Prior supervisory or management experience in a healthcare or senior living environment
• Experience in staff training and professional development preferred
• Strong relationship-building skills with residents, families, and team members
• Proficiency in English (reading, writing, speaking) for effective communication
• Strong documentation and clinical reporting skills
• Basic computer proficiency (Word and Excel required)
• Ability to manage health monitoring systems and ensure consistent delivery of safe care
• Strong organizational skills and ability to adjust priorities based on dynamic resident needs
• Professional representation of the community in all interactions
• Ability to meet state licensure health-related requirements
• Ability to perform essential job functions with or without accommodation
• Successful completion of pre-employment screenings, including background check and TB testing
Essential Functions
Leadership & Management
• Provide direct supervision to all care department team members, including hiring, training, performance evaluations, and corrective action
• Serve as a key member of the community leadership team; support the Executive Director with operational initiatives
• Develop, maintain, and monitor staffing schedules to ensure appropriate daily coverage
• Manage departmental operations within established budget parameters
• Support marketing efforts and build referral relationships within Visalia and the broader healthcare community
• Deliver training related to nursing practices, dementia care, and safety protocols; maintain training documentation
• Ensure compliance with OSHA requirements and California RCFE/Memory Care regulatory standards
Program Oversight & Culture
• Ensure the community’s care philosophy is consistently executed
• Role-model professionalism, resident engagement, and effective communication
• Promote meaningful, individualized resident interactions consistent with personal history, preferences, and abilities
Resident Care & Clinical Operations
• Conduct and oversee comprehensive resident assessments and service plans
• Lead resident and family care conferences
• Coordinate communication among residents, families, physicians, and care teams
• Evaluate changes in condition, implement interventions, and ensure timely documentation
• Perform skilled nursing tasks per scope of practice and community policy
• Oversee medication management, eMAR accuracy, and safe medication storage
• Monitor key health indicators: nutrition, hydration, skin integrity, mobility, fall risk, and pain
• Manage clinical supplies and medical equipment inventory
• Review health records regularly for accuracy, compliance, and resident-specific outcomes
• Administer/monitor TB testing per state requirements
• Ensure resident dignity, choice, safety, and respect remain central in all care delivery
• Provide clinical guidance, mentorship, and coaching to all care staff
• Adjust departmental responsibilities as needed to maintain exceptional quality of care
Compensation
Salary Range: $73,000 – $80,000 annually (DOE)
